Scholarship Application Procedure:

  1. Fill up the application forms available at the Office of the Scholarship Coordinator
  2. Present the following together with the duly accomplished application form:
    • Photocopy of high school card (for freshman applicants)
    • Report of grades issued by the Registrar (for sophomore to senior student applicants)
    • Photocopy of present registration form
    • Certificate of Indigence
