Admission Requirements
Requirements for 2nd. Semester AY 2021-2022:
- Copy of the approved schedule with QR Code
- Vaccination Card for fully and partially vaccinated individuals. If coming from Alert Lever 3 area, present also a Negative Antigen result taken within the last 24 hours
- School ID / Government-issued ID / Certificate of Residence from your Barangay
- Face Mask (No face mask, No entry)
- Partially vaccinated and unvaccinated (Alert Level 2 only) students are required to use face shield in addition to face mask and minor students shall be accompanied by a fully-vaccinated adult. If coming from Alert Level 3 area, present a Negative Antigen (NA) result taken within the last 24 hours. (No face mask, face shield and/or Negative Antigen, No Entry)
- Bring your own ballpen, and alcohol for disinfection of hands
(Submit Original Copies plus 3 sets photocopy/xerox)
High School Graduate of the Old Basic Curriculum (2015 and below)
- High School Report Card (Form 138)
- Certificate of Good Moral Character
- Birth Certificate issued by the Philippine Statistics Authority (PSA) formerly NSO
- 2 pcs. passport size, white background, with name tag (ex. DELA CRUZ, Juan)
Senior High School Graduate (2016 and above):
- SF 9 (Senior HIgh School Report Card)
- Certificate of Good Moral Character
- Birth Certificate issued by the Philippine Statistics Authority (PSA) formerly NSO
- 2 pcs. passport size, white background, with name tag (ex. DELA CRUZ, Juan)
ALS/A&E Passers (Secondary Level):
- ALS Rating (PASSED)
- Certificate indicating that the student is eligible for admission to college.
- Certificate of Good Moral Character
- Birth Certificate issued by the Philippine Statistics Authority (PSA) formerly NSO
- 2 pcs. passport size, white background, with name tag (ex. DELA CRUZ, Juan)
Transferees (In-bound):
- Transfer Credentials
- Certificate of Final Grades
- Certificate of Good Moral Character
- Transfer Certificate
- Birth Certificate issued by the Philippine Statistics Authority (PSA) formerly NSO
- 2 pcs. passport size, white background, with name tag (ex. DELA CRUZ, Juan)
Cross-enrollee (In-bound from other school):
- Permit to Cross-enroll
Returning Students:
- Certificate of Final Grades or class cards from previous semester
Graduate School:
- Transfer Certificate
- Certificate of General Weighted Average (GWA)
- Official Transcript of Record
- Birth Certificate or if married woman Marriage Contract issued by the Philippine Statistics Authority (PSA) formerly NSO
